This form is also known as the Veterans Administration Form 10216.
The Education Liaison Representative of jurisdiction must receive via email a copy of this form, along with an executed copy of VA FORM 22-10215 (STATEMENT OF ASSURANCE OF COMPLIANCE WITH 85 PERCENT ENROLLMENT RATIOS) for the corresponding term, in order to be considered for the 35 percent exemption. For vocational flight schools, the VA Form 22-10217 should be used instead of the VA Form 22-10215.
